Job Functions/Responsibilities:
- Provide care where ever the patient calls home.
- Carry out the plan of care as directed by the patient's physician.
- Work with the physician for medication needs and changes to the plan of care.
- Ability to complete documentation within an EMR System (WellSky).
- Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed.
- When appropriate, teach and counsel family members about care and process for disease/injuries.
- Attend orientation, meetings and CEUs
