Our Social Worker Will:
- Carry out timely social work evaluations, including family dynamics, caregiver abilities, communication patterns, high-risk for suicide, neglect or abuse and plans intervention based on evaluation findings.
- Counsel patient and family/caregivers as needed in bereavement, stress, and other identified coping difficulties. Provide crisis intervention when necessary.
- Provide information and referral services for organization patients and families/caregivers regarding practical and environmental needs.
- Maintain records of social work services utilization and related activities for quality assurance, program development, and policies and procedures review and revision.
- Attend patient care conferences and interdisciplinary group meetings as a member of the interdisciplinary group.
- Provide consultation, education, and support to the interdisciplinary group on psychosocial aspects of end-of-life.
