Heritage Pointe of Warren is seeking a dedicated and experienced Licensed Practical Nurse (LPN) to join our team as the LPN Rehab Unit Coordinator. This is a leadership role and an excellent opportunity for a skilled nursing professional who is passionate about patient recovery and team development.
About the Role:
As the LPN Rehab Unit Coordinator, you will play a crucial role in ensuring that our facility delivers high-quality, patient-centered care while fostering a culture of excellence among the clinical team. The LPN Rehab Unit Coordinator provides direction and leadership to nursing staff to ensure the highest quality of care for all residents.
Key Responsibilities:
- Oversee the delivery of compassionate, evidence-based nursing care that meets the individualized needs of each patient.
- Develop and implement comprehensive discharge plans, ensuring smooth transitions from rehabilitation to home or other care settings.
- Maintain a patient-first approach, ensuring a positive and supportive experience for patients and their families.
- Lead, mentor, and support team of Nurses, QMAs, and CNAs, to ensure high standards of care are met.
- Work closely with Physicians, Therapists, Social Workers, and other healthcare professionals to coordinate comprehensive rehabilitation plans.
- Monitor patient process and implement continuous improvement strategies to optimize clinical outcomes.
- Ensure adherence to state and federal regulations, facility policies, and best practices in post-acute rehabilitation.
Qualifications:
- Current, valid nursing license in the state of Indiana.
- Previous experience in post-acute rehabilitation management is highly preferred.
- Strong leadership, problem-solving, and communication skills.
- Ability to prioritize and adapt in a dynamic, fast-paced healthcare environment.
- A passion for mentoring staff and fostering a positive team culture.
- Excellent organizational and time-management skills.
