Heritage Pointe of Warren is seeking a dedicated and experienced Registered Nurse (RN) to join our team as the Rehabilitation Unit Director for our 20-bed post-acute rehabilitation unit. This leadership role is an excellent opportunity for a skilled nursing professional who is passionate about patient recovery and team development.
About the Role:
As the Rehabilitation Director, you will play a crucial role in ensuring that our rehabilitation unit delivers high-quality, patient-centered care while fostering a culture of excellence among the clinical team. You will oversee daily operations, patient outcomes, staff development, and regulatory compliance, all while working closely with interdisciplinary teams to support the recovery journey of our patients.
Key Responsibilities:
- Oversee the delivery of compassionate, evidence-based nursing care that meets the individualized needs of each patient.
- Develop and implement comprehensive discharge plans, ensuring smooth transitions from rehabilitation to home or other care settings.
- Maintain a patient-first approach, ensuring a positive and supportive experience for patients and their families.
- Lead, mentor, and support team of Nurses, QMAs, and CNAs, to ensure a high standard of care.
- Work closely with Physicians, Therapists, Social Workers, and other healthcare professionals to coordinate comprehensive rehabilitation plans.
- Monitor patient process and implement continuous improvement strategies to optimize clinical outcomes.
- Ensure adherence to state and federal regulations, facility policies, and best practices in post-acute rehabilitation.
Qualifications:
- Current, valid Registered Nurse (RN) license in the state of Indiana.
- Previous experience in post-acute rehabilitation management is highly preferred.
- Strong leadership, problem-solving, and communication skills.
- Ability to prioritize and adapt in a dynamic, fast-paced healthcare environment.
- A passion for mentoring staff and fostering a positive team culture.
- Excellent organizational and time-management skills.
