Position Summary :
The Lead Inpatient Case Manager is responsible for being the "first line" resource for inpatient review staff. This includes questions regarding procedures, training, and processes to assure compliance with Heritage Sierra Medical Group policies and procedures. The Lead assists the Inpatient Case Managers with daily management of inpatient review, auditing functions, and ensuring that all staff provides a level of customer service that meets or exceeds the organization's expectations. The Lead will also carry a small caseload, and responsibilities include determining the appropriateness of hospital and skilled nursing admissions, inpatient review, and discharge planning. Develops a plan of care initiated on the day of admission in collaboration with; the admitting, attending and consultant physicians, and other healthcare practitioners.

The Lead Inpatient Case Manager must perform all duties of an Inpatient Case Manager along with the Duties and Responsibilities listed below :
- Demonstrates the ability to act as a subject matter expert to the inpatient team.
- Provides on-boarding, training and mentorship of inpatient case managers.
- Acts a resource for the Case Manager and Hospitalists on complex cases and issues.
- Monitors staff performance and assures timely follow-up on issues leading to timely discharge and effective execution of discharge plan and management.
- Provides input on case manager performance reviews.
- Reviews census at least twice per day and identifies cases that need intensive follow-up or action.
- Attends rounds daily, provides leadership during rounds on complex cases. Ensures discharge planning and follow-up provided by medical director at rounds are carried out in a timely manner by case management staff.
- Daily interaction with inpatient case managers on cases.
- Provides coverage for team members that are off and assists Manager in workload balancing across the team.
- Works collaboratively with other departments, such as the inpatient coordinators, to ensure optimal inpatient management and discharge planning and follow-up.
- Effectively carries and manages a modified case load as assigned.
- Identifies and escalates performance issues within the team to the manager.
- Ensure the correction of errors / error reports. Communicates with and trains staff on preventing errors.
- All other duties as directed by management.
The pay range for this position at commencement of employment is expected to be between $37.50 and $40.00 / hour; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, skills, and experience. The total compensation package for this position may also include other elements, including a sign-on bonus, restricted stock units, and discretionary awards in addition to a full range of medical, financial, and / or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.
