What are the responsibilities and job description for the Service Coordinator position at Heritage Valley Health System?
Department : Staunton Clinic
Work Hours : Monday through Friday; primarily daylight hours with flexibility to provide clinic / client coverages as needed
To provide coordination of services, linkage / referral, monitoring, service planning, problem resolution, assessment, accessing services, education and advocacy to individuals with mental health / co-occurring disorders and their families who require supports to work toward recovery and increase community tenure.
Requirements :
Bachelors Degree in Psychology, Social Work, Counseling or related field with three years experience working directly with individuals in mental health.
Valid Pennsylvania Drivers license.
Successful completion of applicable clearances as outlined in Human Resources policy HR-106 prior to commencing employment.
Basic cell phone, computer and keyboard skills, including familiarity with Microsoft Office, (Word, Excel and Outlook) and the internet.
Preferred :
Case management experience.
Extensive knowledge of persons with mental health / co-occurring disorders, knowledge of regulatory agencies and payor policies, as well as community resources.
