Position Title: Care Coordinator
Reports To: Area Supervisor
Position Summary
The Care Coordinator at Heroes Home Care plays a vital role in improving the quality of life for elderly clients by ensuring their care needs are met with compassion, diligence, and professionalism. This position involves building strong relationships with clients, families, and caregivers while managing schedules, overseeing caregiver training, and maintaining high standards of care delivery. The Care Coordinator is responsible for scheduling, shift coverage, client satisfaction, and caregiver support, ensuring that no client goes without the care they need.
Key Responsibilities
- Scheduling and Coverage:
- Ensure the schedule for the following week is complete and finalized by Wednesday with no open shifts.
- Respond promptly to missed shift notifications and secure coverage, including covering shifts personally when necessary.
- Assign caregivers to clients, ensuring a good match based on skills and compatibility.
- Caregiver Oversight:
- Conduct caregiver evaluations at 90 days and annually.
- Train new caregivers and ensure their onboarding process is thorough and client-specific.
- Monitor caregiver clock-in and clock-out accuracy.
- Track and coordinate caregiver training needs with the Area Supervisor.
- Client Care Management:
- Manage a caseload of 35-45 clients (approximately 300 care hours).
- Regularly update client files, approve shift notes, and ensure hours are being utilized effectively.
- Conduct follow-ups with clients and caregivers to evaluate progress and satisfaction.
- Collaborate with families and healthcare providers to address client needs and promote continuity of care.
- Educate clients and families on procedures, care plans, and available resources.
- Administrative Duties:
- Submit a daily report to the Area Supervisor summarizing key activities and updates.
- Maintain accurate documentation of all care-related activities.
- Interact with social service programs as needed to support client needs.
- On-Call Responsibilities:
- Be available for on-call duties as required, ensuring all on-call days are covered.
Qualifications
- High school diploma or equivalent required.
- Current CPR/First Aid certification required.
- Minimum of one year of experience in healthcare or a related field preferred.
- Ability to pass a criminal background check and pre-employment drug test.
- Strong attention to detail and excellent communication skills.
- Ability to work independently and as part of a team.
Physical and Travel Requirements
- Physical demands associated with office and light caregiving tasks.
- Some travel required for client visits, caregiver training, or other related duties.
Work Schedule
- Monday through Friday, 8:30 AM to 5:00 PM, with occasional on-call responsibilities.
