Job Description:
POSITION SUMMARY After receiving training from an approved program and listing on the Minnesota Nursing Assistant Registry, works under the supervision of the Charge Nurse and performs many tasks related to resident care. Assists with residents personal care, helping residents maintain their dignity and the highest possible quality of life. MAJOR JOB RESPONSIBILITIES 1. Provides bathing, grooming, dressing and oral care for each resident according to the care plan. 2. Monitors skin condition on residents/patients and reports unusual finding to the Charge Nurse. 3. Assists resident with eating when assistance is needed i.e. tray set up, feeding, and encouragement. 4. Helps manage residents /patients toileting needs by adhering to established routines. Empties Foley drainage bags and monitors patients /residents intake and output if ordered. 5. Assists residents with their mobility i.e. walking, ROM, transferring according to the nursing rehab plan. 6. Functions as part of the care team to provide all other cares i.e. watches patients/residents for safety, serves snacks, escorts patients/residents to activities, X[1]ray, or Lab. May accompany residents on outings. 7. Obtains resident vital signs, heights, and weights as directed. Reports unusual findings to the Charge Nurse. 8. Maintains Room Order during each shift: Makes beds, keeps rooms organized, removes trash and soiled linens at least once per shift. 9. Maintains a clean, safe environment throughout the facility by following current Infection Control protocols. 10. Maintains current knowledge of residents Summary Plan of Care/Care Card. Promptly notifies Charge Nurse or IDT when changes need to be made. 11. Completes all required documentation in Meditech each shift. 12. Stays current with information needed to perform duties by: a. Attending monthly department meetings and educational in-service meetings. b. Acknowledging assigned policies in PolicyStat in a timely manner. c. Completing assigned Relias Modules in stated timeframe. 13. Performs other duties as assigned
MINIMUM QUALIFICATIONS Education: Minnesota Nursing Assistant Training Course and Competency Examination Licensure/Certification/Registration: NAR registration with the State of Minnesota; Current BLS
