Floyd Valley Healthcare in Le Mars, IA has a full-time position (40hours per week) in our Home Health / Community Health Department Manager (RN required). Our Home Health / Community Health department manages an average census of 60-70 patients and serves as the community health and public health of Plymouth County, Iowa.

Responsibilities :
Act as the liaison between the hospital's Administration, the Board of Trustees, the staff and the community.
Responsible for the care provided to our home health patients and for the affairs of the Community Health Department.
Provide leadership to skilled services in Medicare Certified Home Health agency including nursing, home care aides, and therapy services.
Ensuring compliance with Medicare Conditions of Participation.
Grant management of public health services for Plymouth County.
Continue evaluation of community health needs and development of health improvement plans.
Participate in community and professional organizations to enhance home health and public health programs.
Requirements :
Previous management experience is a plus.
Licensed as a registered nurse (RN), in Iowa and / or meets the requirements of multi-state licensure required.
Valid driver's license and proof of insurance.
