POSITION SUMMARY:

This position is responsible for conducting the comprehensive educational and intensive case management services for pregnant and parenting teens participating in the Future Promises Program. This position requires the provision of appropriate assessment, intervention and evaluation of the teens and their children through quality and consistent execution of the program goals and objectives.

ESSENTIAL JOB DUTIES & RESPONSIBILITIES: (Other duties may be assigned)

  • Provide intensive case management for program participants with timely document efforts in the computerized data management system
  • Develop supportive and caring relationship with all program participants and their families
  • Provide case management in a coordinated and culturally sensitive approach
  • Conduct timely individual assessments and screenings for each participant and their child
  • - demographics, depression, self-esteem, nutrition, developmental, etc. as designated by the program with close monitoring of progress in areas of concern.
  • Develop individual care plan with short- and long-term goals with the participant. Develop interventions utilizing community agencies, families, and schools, as needed, to decrease barriers from achieving program goals and objectives
  • Assess and monitor school performance and attendance. Provide needed intervention on problems identified.
  • Assess family/peer relationships. Provide needed intervention to problems identified
  • Assess child development and parent/child relationships. Provide needed intervention to problems identified
  • Evaluate and provide intervention on social or environmental factors that may prevent the participants from achieving program goals and objectives
  • Conduct home assessments and/or hospital visitations as needed.
  • Refer participants to community agencies as needed. Follow-up all referrals
  • Implement the evidence based educational programs with fidelity at the assigned school or community location. Document as required by the program.
  • Assist in the development and coordination of the incentive stores, annual celebrations and recognition activities, community service project, college tour and other activities for families and child’s other parent.
  • Participate in all program evaluation activities. Provide timely program reports and individual assessments and documentation, as directed by the Future Promises staff manager and administration, for program evaluation, grant and corporation reports.
  • Coordinate regular meetings with appropriate school staff and Future Promises Program staff manager to discuss concerns regarding specific participants and the progression of the program to meet the goals and objectives.
  • Meet with the Future Promises Program staff manager, to discuss and receive consultation on the program’s progression in meeting its goals and objectives. Problem solve any issues of concern. Attend and participate in regular team or staff meetings.
  • Assist in program present at various local, regional and national conferences as requested
  • Participate in professional development activities to keep abreast of the latest information that is appropriate to this employment.
  • Assure the programs, their missions and services are consistently presented in a strong positive image to relevant stakeholders, collaborating schools and agencies.

QUALIFICATIONS:

  • Bachelor’s Degree in Nursing, Public Health, Social Work, Health Education/Administration
  • Minimum of three to five years experience in working with minority at-risk children, adolescents and their families in a school, community and home setting. Preferred maternal/child health background
  • Experience in public speaking
  • Experience in community-based health promotion programs
  • Valid Indiana Driver’s License and vehicle insurance

KNOWLEDGE SKILLS AND ABILITIES:

  • Excellent written and verbal communication skills and a keen ability to pay close attention to detail
  • Excellent time management and organizational skills with the ability to be multi-tasked and function effectively under stress and deadlines
  • Knowledgeable of normal child and adolescent growth and development; positive relationship building skills; pregnancy, fetal development and the process of childbirth; adolescent reproductive health including adolescent sexuality development, contraception, and STI/HIV/AIDS; high school graduation requirements, Title IX, and college entrance procedures.
  • Knowledgeable of Positive Youth Development approach and Motivational Interviewing
  • Ability to work independently without daily monitoring of time and completion of duties
  • Energetic and Self-directed
  • Able to occasionally work longer hours, evenings or weekends as the duties dictates
  • MS Word and Excel
