Job Summary:
The Registered Nurse (RN) will provide skilled nursing care to home health patients, ensuring high-quality, compassionate, and individualized care. The RN is responsible for patient assessments, care planning, wound care, medication management, and coordinating with physicians and caregivers.
Key Responsibilities:
✅ Conduct comprehensive patient assessments and develop individualized care plans.
✅ Administer medications, injections, wound care, and IV therapy as needed.
✅ Monitor and document vital signs, progress, and response to treatment.
✅ Educate patients and families on disease management and home care techniques.
✅ Coordinate care with physicians, therapists, and home health aides.
✅ Ensure compliance with state and federal regulations (Medicare, Medicaid, etc.).
✅ Supervise LVNs and home health aides as needed.
✅ Maintain accurate electronic documentation and submit timely reports.
Qualifications:
✔ Current RN license in Texas (unrestricted)
✔ Strong clinical skills, critical thinking, and communication
✔ Knowledge of OASIS documentation & Medicare guidelines (preferred)
✔ Reliable transportation & valid driver’s license
Licensed Vocational Nurse (LVN) – Home Health
Location: Starr County, TX

Job Summary:
The Licensed Vocational Nurse (LVN) provides skilled nursing care under the supervision of an RN. The LVN will assist with patient care, medication administration, wound care, and monitoring of health conditions.
Key Responsibilities:
✅ Provide direct nursing care to home health patients.
✅ Administer medications, injections, and treatments as ordered by the physician.
✅ Perform wound care, catheter care, and other skilled nursing tasks.
✅ Monitor and document vital signs, symptoms, and patient progress.
✅ Educate patients and caregivers on health conditions and care techniques.
✅ Communicate with physicians, RNs, and the home health team.
✅ Ensure compliance with home health regulations and agency policies.
Qualifications:
✔ Current LVN license in Texas (unrestricted)
✔ 1 year of home health or clinical experience (preferred)
✔ Strong communication and hands-on nursing skills
✔ Experience with electronic documentation & home health software (preferred)
✔ Reliable transportation & valid driver’s license

Pay Rate: Competitive, based on experience
Schedule: Flexible shifts available
