We are dedicated to providing high-quality, patient-centered care to our residents. Our Continuing Care Center offers a unique and supportive environment for individuals who require specialized care.
The center has 120 beds and provides three distinct levels of service in semi-private rooms: Subacute care, Skilled nursing care, and Long-Term Care.
This role will be responsible for assessing, planning, implementing direct clinical care to assigned patients on a per shift basis, and unit level. The role will also supervise staff and ensure the competency of clinical staff.
Responsibilities:
- Affirm responsibilities include overseeing day-to-day nursing operations, supervising nurses, and assigning tasks to achieve optimal care outcomes.
- Clinical judgment is required to identify changes in patient conditions, develop comprehensive plans for patient care, and allocate resources effectively.
- Effective communication with team members, physicians, and families is essential to coordinate seamless care transitions and foster collaborative relationships.
- Continuous quality improvement involves analyzing data, identifying trends, and implementing enhancements to elevate care standards and optimize resource utilization.
