Description
Provide assistance within the Health Services Department as directed, performing nursing tasks such as completing duties for assigned individuals as identified on client’s health care plan, medication administration, overall assessment of health condition, and working with Hi-Hope team on treatment planning. Assisting with nursing needs for HHSC as needed and other duties as required.
Requirements
· Complete nursing tasks for assigned individuals and complete timely documentation of those tasks per Medicaid Waiver funded Nursing Services guidelines.
· Weekly review of CareSuite QuickMAR, Therap and paper MAR’s documentation to ensure accuracy and completeness of documentation for assigned individuals. Notify the Health Services Manager/Director of Quality Improvement of any concerns.
· Review health care plans for assigned individuals and complete clinical tasks
· Assist with medication administration for assigned individuals as needed.
· Perform triage and minor first aid to assigned individuals as needed.
· Assist with transporting assigned residents to medical appointments when necessary.
· Review documents following medical appointments and ensure timely follow-up of recommendations for assigned individuals.
· Maintain updated list for assigned individuals of Medical Devices, Seizures, Dietary Restrictions, Diabetes, Choking or Fall Risk, and Impairments.
· Attend meetings as required.
· All other duties as assigned.
SKILLS & ABILITIES
Minimum Qualifications
Education: High School Graduate or General Education Degree (GED). Graduate from a Georgia approved LPN school.
Experience: One to two years related experience
Certifications & Licenses: Proof of Georgia Licensed Practical Nurse License and current Driver's License from the State of Georgia required.
Computer Skills: Must be experienced with using Windows Microsoft Software, knowledgeable with, creating Excel spreadsheets, using electronic medication administration records, and using electronic medical record database.
Other Requirements
Must be 21 years old, able to pass a pre-employment physical and capable of lifting a minimum of 50 pounds; have a negative drug screening and a tuberculosis (TB) screening or X-Ray; and, have an acceptable motor vehicle record with no more than two moving violations in a three year period within the last five years of a seven year motor vehicle record report, vehicle liability insurance and an acceptable criminal record check. Must have reliable transportation.
