Case Manager
SUMMARY:
Promote recovery and community integration for adults with mental illness, respecting their right to person
centered planning and least intrusive alternatives, while also providing support services. Responsible for
assessment, treatment planning, monitoring, linking and coordination for person served. Performs necessary
liaison, advocacy and consultation work with ancillary agencies such as: Department of Health and Human
Services, Courts, Foster Care Providers, Guardians, and Families. Provide periodic reviews to ensure
appropriate services are being provided, and treatment plans are reflective of services. Responsible for
continuous development of treatment plans, providing guidance, and advocating for person served.
QUALIFICATIONS:
1. Bachelors or Masters Degree required in Social Work, Psychology, or other related Human
Service field.
2. State of Michigan Registration as a Licensed Bachelors (LBSW or LLBSW) in Social Work
preferred.
3. Proficient computer skills necessary for data entry into electronic medical record, with
experience utilizing Excel & Microsoft Word programs.
4. Appointment is subject to driver’s license, criminal, and professional background check.
Must be at least 18 years of age, possess a valid driver's license, and provide own
transportation.
SALARY: Exempt | Union | $43,305.60 - $58,947.20
WORK HOURS: Full Time | Regular |40 Hours a Week
