What are the responsibilities and job description for the Physical Therapist or Physical Therapy Assistant position at Hiawatha Homecare?
Qualifications:
- Bachelor's (Preferred)
- Physical Therapy or Physical Therapy Assistant License (Required)
- CPR Certification (Preferred)
Primary Responsibilities:
- Provides physical therapy to home care patients and case management when indicated.
- Responsible for timely and accurate communication and documentation relative to physical therapy services.
- Promotes and develops physical therapy services in homecare.
Specific Job Duties - includes, but not limited to:
- Evaluate and treat patients suffering from physical disability due to injury, disease, or surgery in a home care environment.
- Performs initial start of care assessment and all related forms.
- Develop, revise and, execute an appropriate care plan for restorative and maintenance patients.
- Collaborate with physicians, case managers, nurses, occupational therapists, physical therapy assistants, home health aides, and office staff to ensure consistent levels of care.
- Monitor patient progress and document accordingly.
- Educate patients, families, and caregivers regarding the proper use of medical equipment or adaptive equipment.

Pay: $30.00 - $60.00 per hour, depending on type of license & experience
