Job Overview
The Community Outreach Worker will build strong community relationships, drive client engagement, and promote agency services. This role involves proactive marketing, managing client records, supporting quality assurance, and ensuring effective communication between staff and clients. The worker will also maintain a personal caseload and provide team support when needed.
Scope of work
- Conduct outreach efforts to attract a minimum of 3 new clients per week across various agency programs.
- Collaborate with the marketing team and attend events to promote the agency's services.
- Create and maintain client charts, including completing referral forms, legal packets, and pre-screening forms.
- Create goal cards for clients in Noteable and assign clients for assessment.
- Input client data into tracking spreadsheets to support the Compliance team during audits.
- Conduct in-person orientation sessions for new clients during the pre-authorization process.
- Ensure legal documents are signed accurately and engage clients to maintain involvement.
- Assign clients to QMHPs once authorized and ensure smooth handoffs.
- Conduct weekly quality assurance (QA) calls with clients.
- Bill one hour for each client care coordination effort through QA calls.
- Address concerns from QMHPs or Supervisors regarding client care and performance.
- Maintain a personal caseload of 20 hours per week.
- Achieve a minimum of 20 hours of billable services through client sessions or case management duties.
- Fill in for client sessions during QMHP absences (e.g., vacations or leaves).
- Assign clients to QMHPs as approved and manage warm handoff processes.
- Conduct New Hire Orientation for new QMHPs, including shadowing sessions.
- Coordinate with Supervisors to address internal investigations or client concerns.
- Attend weekly leadership meetings (2 hours) : 1 hour with the Director and 1 hour with clinical supervisors to ensure smooth operational flow.
