What are the responsibilities and job description for the Medical Office Assistant Psychiatric Navigator position at High Country Community Health?
Job Summary and Responsibilities:
This position will serve as the patient navigator for the behavioral health medical provider
(psychiatrist and/or psychiatric mental health nurse practitioner). The patient navigator will
provide support and assistance in psychiatric medication management at
HCCH. In addition, this position will support the behavioral health team as needed.
This position will also be a response to QI patient chart auditing for quality
improvement by assisting the QI Director with manual chart audits. These chart audits will be
utilized to determine documentation and coding is appropriate.
Responsibilities include:
- Contacting patients to verify psychiatry appointments
- Introducing patients to the process of virtual psychiatry appointments
- Coordinating clinic schedules to ensure patients are roomed, triaged, and ready for psychiatry appointments at their scheduled time
- Triaging patients, based on need, if there is an active waitlist
- Training staff across sites on psychiatry procedures and documentation
- Completing prior authorizations, as needed for the Behavioral Health program
- Assisting with scheduling patients for psychiatry intakes and follow-up appointments
- Attending team huddles
- QI patient chart auditing
- Ability to effectively communicate with a diverse population including patients, coworkers, and community stakeholders
- Experience, education, or training in psychology, social work, counseling, or a similar discipline
- Ability to interview patients with tact and cultural sensitivity in order to obtain necessary information to make appropriate referrals and follow-up
- Ability to learn and utilize all relevant features of the Electronic Health Record in documentation of clinical work
- Excellent time management and organizational skills
- Self-motivated and self-guided
Qualifications:
Graduated from an accredited college or university with an appropriate degree relevant to the
role. A minimum of two years’ experience in medical or behavioral health setting is preferred.
Demonstrated ability to collaborate effectively in a team setting.
