Summary
As a member of the Hospice interdisciplinary team, the primary focus of the Bachelor Social Worker is to respond to the community resource needs of the patients and their families. The hospice social worker provides emotional support to patients/families, hospice staff, and volunteers as needed while acknowledging and respecting their beliefs, culture and values related to life’s meaning including suffering and loss and a desire to receive support. The Bachelor Social Worker role allows for social workers to be supervised by an MSW/LMSW/LCSW and allows the Bachelor Social Worker to gain experience as they continue to develop their social work skills and continue their social work education.
Job Requirements
· Operates under the direction of the Clinical Care Manager.
· Participates in IDT meetings.
· Provides in-service education to volunteers.
· Maintains documentation of social work services in patient’s medical records according to agency policy.
· Acquires on-going education and training through conferences, meetings, and current literature.
· Elicits and listens to the story of patient, family and friends.
· Knowledge of the hospice benefit/Medicare, complete physical assessment, completion of all pertinent paperwork, and communication of patient status to the hospice team.
· Participate in hospice and community health programs as requested to promote the growth and understanding of the hospice concept.
· Performs other duties as assigned, consistent with skills and training and the mission and goals of the agency.
· Contributes toward the achievement of company’s strategic and operational objectives.
- Undertakes regular self-assessment and training to assure continued professional growth and development, including attending conferences and training as requested.
Qualifications
· Bachelor of Social Work degree.
· Hospice experience and/or equivalent education is preferred.
· Excellent advocacy and collaboration skills.
· Knowledge of community resources and the dynamics of terminal illness, grief, and bereavement.
· Demonstrate capability identifying family dynamic and familiarity of death and dying concepts.
· Ability to work in a collaborative team relationship.

Pay: $23.00 - $33.00 per hour
