Hours: M-F; 8am-5pm (and on-call rotation)

As an ACT Case Manager (Recovery Specialist), you’ll work directly with consumers in their homes and communities, empowering them to achieve their goals and improve their quality of life.
What You’ll Do:
- Develop and maintain support for consumers, helping them reach their goals through community support.
- Assist with the development, implementation, and maintenance of consumer treatment plans.
- Deliver rehabilitative services, engaging consumers in activities that promote symptom management and higher levels of functioning.
- Provide mobile crisis services in collaboration with your team, improving outcomes such as reducing hospitalizations and stabilizing housing.
- Function as a contributing member of the ACT multi-disciplinary team.
- Support consumers by coordinating psychiatric hospital admissions and discharges.
- Provide after-hours on-call crisis services as part of a rotation.
- Travel to deliver services and respond to emergencies.
- Build professional relationships with stakeholders and fellow team members.

Requirements:(1) Have at least a Bachelor’s degree or be equivalently qualified by a combination of education and work experience in a human service field;
(2) possess demonstrated interpersonal skills, ability to work with persons with serious mental illness, those who experience significant functional impairment and/or are at risk due to mental illness, and the ability to react effectively in a wide variety of human service situations;
(3) pass a KBI criminal history background check, DCF Child Abuse/Neglect and Adult Abuse/Neglect Registry check, professional background checks and completion of a state approved standardized basic training program;
(4) meet the specifications outlined in the CMHC licensing standards in regard to any ongoing requirements; and
(5) pass motor vehicle screen. Must demonstrate competency in the use of technology including computers, various software applications, printers, scanners, telephones, etc. Must also demonstrate a commitment to continuous improvement, to include understanding and application of technology (hardware, software, equipment and processes).
