Location: Hays, KS (with travel to outreach clinics – center vehicle provided)
Pay: $23.17 per hour
Job Type: Full-time, In-Person
Schedule: Monday-Friday, 8:00 AM - 5:00 PM

What You’ll Do:

  • Act as the primary point of contact between HPMHC, primary care providers, hospitals, and clinics in the community.
  • Manage and coordinate patient referrals to HPMHC, facilitating seamless transitions to mental health services.
  • Educate patients and providers about the referral process, ensuring a smooth journey through healthcare services.
  • Coordinate schedules for HPMHC providers and specialty clinics, ensuring efficient use of resources.
  • Serve as the point of contact for the TBHN Expansion Grant, which will provide telehealth psychiatric services to rural hospitals.
  • Cultivate strong, professional relationships with healthcare partners, building trust and fostering cooperation.
  • Maintain documentation and records in compliance with HPMHC policies and ensure timely communication with all stakeholders.
  • Participate in required training and team meetings to stay updated on mental health practices and regulations.
  • Travel to outreach clinics as necessary, supporting rural communities with critical mental health services.

Requirements:
  • Current licensure as a Licensed Practical Nurse (LPN) by the Kansas State Board of Nursing.
  • Minimum of one year of nursing experience, preferably in psychiatric or mental health nursing or physician office practice.
  • Experience working with individuals with serious mental illness or those at risk due to mental health conditions is highly preferred.
  • Strong interpersonal and communication skills, with the ability to work effectively with diverse stakeholders, including healthcare providers, patients, and community partners.
  • Proficient in the use of technology, including healthcare management software, computers, printers, scanners, and telecommunication tools.
  • Ability to work independently and in collaboration with a multidisciplinary team.
  • Knowledge of medical terminology and healthcare procedures, with a focus on mental health.
  • Ability to effectively manage multiple tasks and prioritize work.
  • Valid driver’s license and a driving record that meets agency insurability requirements.
