Aftercare Coordinator
Program & Location: Women's Addiction Treatment Center(WATC)/New Bedford
Pay Range: $22 - 27 (experience and education dependent)
Status: Full-Time
Shift: First Shift, Monday - Friday

High Point has programs offering Section 35 services for individuals that have been civilly committed to Substance Use Treatment. Services include Acute Treatment Services (detoxification) and Clinical Stabilization Services. These programs are staff secured units, not locked.
High Point & Affiliated Organizations is a health and human service agency whose mission is to treat and prevent substance use disorders and mental illness. High Point has programs located throughout Southeastern Massachusetts offering a full continuum of care for substance use and mental health treatment, including inpatient, outpatient, residential, and community-based services. Programs and services also assist survivors of abuse, violence, and families experiencing homelessness. High Point believes that everyone has inherent goodness, worth, and dignity. Aftercare Coordinator Duties & Responsibilities:
- Assist patients with identifying aftercare and continued treatment services and support recovery;
- Assist patients by providing information regarding community supports, family supports, and coordination of services;
- Participating in setting the therapeutic milieu of the unit, and remaining calm in a crisis;
- Facilitate patient groups to discuss aftercare/service coordination, family support, and relationships;
- Complete referrals for aftercare based on patient needs and identified preferences to community-based supports, sober housing, continued 24hour treatment settings, and outpatient;
- Coordination of MAT services following discharge with identified provider, counseling, and ensure dosing information if effectively communicated;
- Advocacy regarding patient’s current treatment, and aftercare;
- Documentation of family meetings and maintain monthly treatment;
- Excellent interpersonal skills to assist the staff on the unit such as providing psych educational groups, facilitating peer led groups, and assisting with other documentation as needed;
- Maintenance of proper boundaries with patients;
- Provide information on area support groups and articles on addiction;
- Attends required meetings and mandatory trainings;
- Knowledge of treatment services, community supports, and continuum of care;
- Ability to effectively communicate needs of patient with referral agencies;
- Other duties as assigned by Program Director of Clinical Services.
Aftercare Coordinator Qualifications:
- Minimum of a high school degree or equivalent GED;
- Minimum of (1) year supervised counseling experience in substance use treatment or closely related field;
- Prior substance abuse experience preferred.
Salary : $22 - $27
