What are the responsibilities and job description for the Mental Health Case Manager LAC / LSW Preferred position at High Point Partial Care?
Job Description:
As an integral part of our growing talented and energetic team, the Mental Health Case Manager will provide services to adults with mental illness in a Partial Care setting, with the goal of helping clients to achieve their recovery goals and to remain stable in the community.
Duties:
- Facilitate group counseling sessions addressing topics such as psychoeducation, peer support, interpersonal skills, recovery strategies, etc.
- Develop collaborative individualized recovery plans with clients
- Implement interventions and monitor responses to support symptom management and improved mental wellness
- Complete comprehensive Mental Health intakes with MH clients
- Provide personal 1:1 support, and assist with de-escalation/crisis-intervention as needed
- Coordinate with clients’ adjunct services for continuity of care, including family members, community workers, group homes, hospitals, DCP&P/Probation, Social Services, etc.
- Maintain client records, including daily group notes and weekly progress notes

- Excellent time management and organizational skills
Qualifications:
- Active NJ licensure including LSW, LAC - Preferred

- Bachelor’s Degree in related field of study (Psychology, Social Work, Counseling, etc.
–OR–
- Associates Degree with work experience in the mental health / human services field
- Experience in mental health and co-occurring treatment
- Ability to conduct thorough bio-psycho-social assessments
Hours:
- Monday through Friday, 8am to 4pm
- Full-time only. No evening or on-call hours
Salary:
$38,000 - $55,000 (with Licensure)

- Clinical supervision towards LCSW/LPC licensure is included free of cost and conducted on-site on a weekly basis for all staff members pursuing their next clinical licensure. Staff members currently pursuing a master's degree are also eligible for internship supervision as well.
