PT LPN Friday, and every other weekend w/ the possibility to pick up additional shifts if available.
The primary purpose of this position is to provide each community resident with routine daily nursing care and services in accordance with the resident’s assessment and care plan.
Position Details:
- LPN with current Illinois nursing license in good standing.
- Recent experience in a fast-paced environment is desirable.
- Should have excellent assessment, clinical and supervisory skills.
- Should be familiar with the medication pass and documentation requirements. Assisting
- Collect information about conditions and treatment plans from caregiver team.
- Other duties as assigned by Wellness Director,

Requirements:
- Current, valid IL state certification and in good standing.
- High school diploma or GED.
- Successful completion of a nursing assistant program.
- Current on the IDPH HCWR Registry
- Ability to perform physical tasks typically performed in CNA work, including lifting up to 25 lbs.
- Valid CPR (recently expired accepted, we will get you recertified)
