SEMCOA operates four emergency assistance congregate family shelters and additional scattered sites that provide supportive services to families and assist them with securing permanent housing. SEMCOA also provides residential recovery home treatment to individuals with substance use disorders, as well as transitional and permanent housing stability and support programs. Our portfolio also includes two graduate sober houses, the Family Preservation Program, as well as Greater Brockton and Greater Attleboro Taunton Healthy Families programs. Case Manager RequirementsMust present as courteous and professional at all timesDemonstrated flexibility and ability to perform multiple tasksMust have excellent communication skills, including the ability to organize ideas in logical andclear fashionKnowledge of basic computer skills and software programsMust have the ability to work with others in cooperative and collaborative mannerAn understanding of the relationship between substance use and mental health issues and / or self help programsUnderstand and maintain therapeutic boundariesReinforce rules in a respectful mannerComply with Federal Regulations, 42 CFR Part 2, Confidentiality of Substance Use Disorder andHIPAA RegulationsTB / Flu and Covid19 AssessmentCertification in the HPAO approved de-escalation programCPR CertificationSuccessful completion of all trainings upon hire and annually as requiredCase Manager Duties & ResponsibilitiesMaintain safety and security of residents / facilityEnsure timely contact of Program Manager / Authorities in event of a crisis / emergencyUnderstand and follow established referral procedure and schedule intakesFacilitate intakes of new residents including setting up new record and completion of intakepaperworkCompletion of Initial Individual Service Plan (ISP) with residentReferrals for needed services to appropriate agencies / providers including but not limited to : DTA,SSA, DCF, Medical / Mental Health agencies, etc.Maintain accurate case files, ensures required Authorizations to release / obtain information aresigned by residents, prepares legible case management notes of each session, documents referrals on designated form, updates ISP's monthly and ensures periodic evaluations are done timelyAccurately record resident attendance at meetings, appointments, groups, etc.Meet with each resident on a bi-weekly basis to review / evaluate progress in treatment / recovery to determine if current needs are being appropriately and satisfactorily met; to revise / update theresident's Individual Service Plan as indicated, including the establishing of additionalgoals / objective and the steps to achieve these goals / objectivesAttend monthly case review with Program ManagerAttend clinical supervision, required staff meetings and trainingsEnsure timely completion of Admission / Discharge satisfaction surveys by residentsHouse driving / transportationAbility to work with underserved and disenfranchised familiesComfortable working in diverse environment and using translation and interpretation toolsTransport clientsAll other duties as assigned by the Program ManagerCase Manager QualificationsHigh School Diploma or GED and one-year experience in substance use treatment field preferred
