Patient Resource Navigator Qualifications
Bachelor's Degree level degree in Psychology or a related human services field (preferred), or equivalent experienceBi-lingual / Multi-lingual applicants are strongly encouraged to applyExcellent interpersonal and communication skills while able maintaining professional boundariesMaintain mental alertness and attention to high detailMaintain a neat and professional appearance and provide excellent customer service as neededFollow established security protocols while using initiative and good judgement without biasPatient Resource Navigator Duties & Responsibilities
Help to support an open access model for screening and assessments for mental health, substance use, and co-occurring treatment services within the CBHC service modelProvide initial screenings and assist patients and families with accessing CBHC servicesAssist patient with intake paperwork and ensure proper release and consent forms are completeParticipates in patient outreach and engagement to support access to treatmentCoordinates patient transportation services, as neededProvide care coordination and discharge planning activities to support transitions of careMaintain and document collateral contacts with other providers as needed, including, but not limited to primary care physicians, probation, parole, DCF, family members, natural supports, etcCoordinate with local Emergency Services Provider for concerns related to imminent psychiatric risk and hospitalization, referral support for CBHC core services, and follow-upEngage in community outreach activities as neededPerform other duties as assigned; including providing security support for the program to monitor patient and staff safety as part of their dutiesPatient Resource Navigator Requirements
Must present as courteous and professional at all timesGood case management skillsExperience working in the mental health / substance use fieldDemonstrated flexibility and ability to perform multiple tasksMust have excellent communication skills, including the ability to organize ideas in logical and clear fashionMust have the ability to work with others in cooperative and collaborative mannerTB screeningCertification in the HPTC approved de-escalation programStrong and effective verbal, written and organizational skillsKnowledge of basic computer skills and software programs including the ability to learn, use and train HPTC`s electronic medical recordsAbility to provide leadership and program development within an outpatient siteKnowledge of major clinical therapeutic models and behavioral health best practiceStrong organizational, verbal and written communication skills
