Job Summary:
The Continuing Care Coordinator is an integral member of the admissions and business development team. The individual shall serve as the principal liaison among external referrers, High Watch clinicians, and High Watch Outreach staff. The primary focus of the position is to ensure that referrers receive timely, thorough feedback about the guests they have referred to High Watch and that the Outreach team is informed about those discussions.
Job Duties
· Understand the admissions, clinical and aftercare processes through continuous training and completing an orientation of programs at High Watch
· Meet and build relationships with aftercare partners and referrers
· Leverage relationships with referrers to increase admissions
· Schedule, facilitate and/or participate in telephone discussions between High Watch clinicians and referrers to update referrers on the status of the individual they referred. This would include but not limited to doctors, social workers, discharge planners and emergency room personnel.
· Schedule, facilitate and/or participate in telephone discussions between High Watch aftercare staff and referrers to update referrers on the aftercare plan of the individual they referred. This would include but not limited to doctors, social workers, discharge planners and emergency room personnel.
· Develop a working relationship with the High Watch clinical, aftercare, and outreach teams.
· Provide and recommend input for marketing materials to improve department performance.
· Attend conferences for doctors and other medical staff when appropriate.
· Keep detailed records of all contacts.
· Build and maintain positive working relationships with referrers
· Maintain knowledge of new developments in the treatment of addiction, anticipating potential negative and positive impacts on the business and adapting strategy accordingly.
· Develop strategies for increasing opportunities to meet and talk to contacts in the medical and healthcare sector
Job Specifications

Minimum Qualifications:
Bachelor’s Degree in a field related to Behavioral Health or at least three years of full-time Behavioral Health work experience, or marketing experience.

1. Knowledge of software programs for personal computer use, especially word processing, email, and spreadsheets.
2. Knowledge and experience of the range and type of services available to substance abuse clients.
3. Ability to prepare and implement presentations.
4. Strong problem-solving skills and the ability to carry out assigned projects to their completion.
5. Strong interpersonal skills, including the ability to communicate effectively both verbally and in writing, establishing and maintaining effective working relationships, gaining cooperation and resolving conflicts.
6. Ability to plan and organize workload, manage deadlines of him/herself.
7. Ability to maintain and demonstrate ethical professional standards and demeanor, including the confidentiality of employee and client data, client boundaries, and other sensitive information.
8. Demonstrated competence with, and continued commitment to respect for cultural differences and diversity, protecting client rights.
9. Recognition of the limits of one’s own professional expertise and the willingness to seek supervision and/or support, as needed.
10. Make organizational decisions with the clients’ and agency’s well-being and best interests in mind.
