Description
Provides rehabilitation programming such as; but not limited to: scheduling outings, documenting outings, and assuring destinations are safe for patients, and following staff to patient ratios. Reports to the Manager of Environmental and Patient Safety.
Duties and Responsibilities
- Works with the treatment team to help provide community integration to patients once they obtain authorization by the treatment team. Will abide by court orders and outing recommendations and restrictions by the treatment team.
- Partner with other agencies to promote and provide services, activities and events for community service opportunities.
- Completes documentation regarding community integration programming and community service.
- Ability to see and address emerging safety issues while out in the community with patients and staff.
- Completes patient orders as needed and helps with patient inventories.
- Assists on transports, work program, and community integration as needed.
- Other duties as assigned.

Education and Experience:
High School diploma or equivalent.
Experience in hospital operations preferred.
Language Skills:
Ability to read and interpret instructions, documents and follow procedures. Ability to write routine reports and speak effectively with patients and their families, employees and public
Reasoning Ability:
Ability to solve practical problems and deal with a variety of variables as needed.
Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Certificates, Licenses, Registration:
Valid West Virginia driver’s license required.
Other Skills and Abilities:
Ability to work flexible hours. Ability to work with children, adolescents or adults experiencing mental health disturbances or dual diagnoses

Incumbent will be expected to participate as necessary in the physical restraint of patients, have the ability to respond to rapidly escalating patient situations, and adapt to the many varied resultant physical posturing as a result of such circumstances after Crisis Prevention Intervention (CPI) training. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
