Job Purpose
The Patient Navigator, Medical Records Specialist, will be responsible for organizing, overseeing, and protecting patient health information data which includes symptoms, diagnoses, medical histories, tests results, and procedures. This professional ensures that traditional and digital health data maintains its quality, accuracy, accessibility, and security, while entering patient information into the computer databases to comply with federal legislation mandating the use of electronic health records. This position is responsible for responding to records requests and transmitting paper and electronic health records in accordance with Health Insurance Portability and Accountability Act Privacy Rules and security requirements.
Qualifications:
Education
- High school diploma required
Experience
- Medical office training and experience preferred.
- Licenses/Certifications Required
BLS certification required.
Position Responsibilities:
- Perform all duties within HIPAA regulations
- Compiling, organizing, maintaining, and protecting confidential health records
- Enters and maintains information in the electronic health record (EHR)
- Monitors information in the EHR for accuracy
- Ensures that health care professionals can access patient’s health information when and where they need it.
- Continually monitor the fax inbox to attach patient records in a timely and accurate manner.
- Scan traditional patient records and attach in the Electronic Health Record in a timely and accurate manner.
- Ensure timely turn around to appropriate patient records requests, following HMC policy and procedures related to health records.
- Crosstrain and assist in the front office with patient navigation as necessary.
- Assist administration as needed.
- Other duties as assigned.
Technical/Computer Skills:
- Proficient in electronic health records, Microsoft Office, and iOS operating systems.

Excellent communication skills and strong customer service orientation. Demonstrated enthusiasm for wellness-based practice and incorporation of faith/beliefs in patient’s health practices.
