General Requirement :

The Unit Clerk / Central Supply. Assists the licensed nursing staff in performing a variety of duties in caring for residents in the facility. The Unit Clerk / Central Supply is expected to have knowledge of policies and procedures for providing bedside and out of bed care.

Responsibilities : I. General

  • Keeps confidential all knowledge of resident problems and / or conditions.
  • Assists new employees in following established facility policies and procedures.
  • Participates in facility education programs as assigned.
  • Participates in resident care conferences and other facility meeting as needed.
  • Participates in individualized plan of care for each resident assigned.
  • Greets residents by name and speaks to residents and families in a kind, thoughtful manner.
  • Works with cooperative spirit.
  • Reports to work in a clean, neat clothing and consistently adhere to dress code.
  • Adheres to facility infection control practices.
  • Offers suggestions to the Charge Nurse that in your opinion would upgrade the quality of care and participate in overall resident care planning.
  • II. Specific Responsibilities

    All residents’ call bell should be answered promptly by all staff members despite role in the facility

  • Maintains central supply par levels.
  • Delivers needed supplies to units.  Maintains unit par levels and cleanliness of all supplies.
  • Makes resident appointments and transportation
  • Prepare residents rooms for admissions, names on residents’ doors,
  • Scans in documents and uploads to resident charts Consultations must be uploaded in 2 weeks.
  • Maintains paper file in medical record room per protocol of older residents.
  • Clears medical record room of old files.
  • Assists with retrieving files as needed
  • Ensures that new admission paperwork that has been scanned is destroyed per protocol.
  • Assists with meals on unit, assists with transportation, toileting
  • Answers phones, assists families as needed
  • Provides pertinent information to nurses and Unit managers
  • Introduce self to new admissions, act as liaison to staff of resident concerns.
  • Assists Human resources with filling the staffing schedule
  • Providing Direct Care (as certified) (including but is not limited to)

  • Personal hygiene—showers, tub baths, sponge baths, etc.
  • Incontinence care in order to keep resident clean and dry.
  • Transfers in and out of bed, lift, turn and positions residents.
  • Oral hygiene and denture care, as needed.
  • Toileting, bedpan / urinal use and empties, clean and sanitizes.
  • Maintains privacy and personal dignity of each resident.
  • Nail and hair care.
  • Assists with activities according to tolerance level, M.D. orders and accountability record.
  • After meal care.
  • Weights as needed.
  • Preparing residents for meals (as certified) :

  • Distributes clothing protectors as resident desires.
  • Serves trays and returns them to the food truck after each meal.
  • Feeds and / or assists residents who need help.
  • Serves between nourishments and provides fresh water to residents.
  • Checks to insure that all residents eating have been served a tray.
  • Wipes tables clean after meals.
  • Records food / fluid intake for residents after each meal.
  • Records intake of nourishments.
  • Offers hs snacks to all residents & intake.
  • Documenting :

  • Completes accountability records as per facility policy.
  • Records information as needed to complete any incident or accident report.
  • Transporting (as certified) :

  • Escorts residents :
  • To clinic and physician’s offices outside of facility, as assigned.

  • To various programs within the facility.
