Complete daily documentation on program members
Responsible for ensuring that all documentation is in compliance with DHR, Managed Care, CARF and Medicaid guidelines as per policy
Facilitate Peer Support recovery groups.
Develops individualized and comprehensive plan of care based on assessment and individual’s level of need and functional ability.
Ensures individuals are receiving the appropriate level of care as determined by treatment plan.
Monitor individual’s progress towards goals.
Provides follow up and referrals for those not attending.
Responsible for ensuring records are closed per HR policy when it is determined the individual no longer needs treatment.
Responsible for ensuring that all services are authorized.
Responsible for carrying out all clinical duties per policy and procedures.
Utilized good clinical judgment to provide appropriate individual care.
Completes CONNECTS and treatment plans.
Responsible for ensuring ISP Reviews are completed per policy.
Work closely with CPS and other staff to ensure success of the program, organize events, projects, and outings.
Responsible increasing Peer Support Average Daily Attendance
Provides Peer Support individuals with case management.
Responsible for transporting as needed.
Maintain all training necessary to operate a state vehicle and transport individuals.
Establishes professional and ethical relationships with individuals, families, and coworkers including maintaining therapeutic professional boundaries.
Exhibits good communication skills (oral, written and active listening)
Communicates suggestions and/or complaints through appropriate channels.
Displays flexibility, cooperation and a positive attitude.
Participates in the Programs Performance Management Evaluation.
Participates in the multidisciplinary team to coordinate services with treatment team.
Completes assigned tasks in a timely manner.
Requests directions and supervision when appropriate.
Adheres to safety and infection control guidelines.
Reports to work on time and as scheduled.
Follows established policies and procedures for calling in for unscheduled absences.
Uses initiative and judgment to assure that circumstances requiring attention are referred to the appropriate team member.
Adheres to facility dress code.
Performs other duties as directed by Outpatient Managers.
Must complete all mandatory training during fiscal year
