JOB SUMMARY: Highland Rivers Behavioral Health offers services for individuals in the community based residential programs meet goals for stable housing.
DUTIES AND RESPONSIBILITIES:
- Provide 1:1 service for individuals on caseload, documenting all interactions in ECR
- Complete monthly apartment inspections, addressing any needed repairs within 30 days.
- Collect, compile, and submit data required for program evaluation and grant reporting under contract.
- Update GRID/MEIRs at least monthly.
- Maintaining accurate rent calculations (at least annually unless there’s a change in income).
- Complete CONNECTs and treatment plans.
- Collect rent from individuals on caseload monthly, delivering payments to RRSS admin assistant.
- Assist individuals with purchasing needed items for apartments, scheduling maintenance as needed.
- Assisting individuals with linkage to appointments, recommending services as needed.
- Other duties as assigned.
