Must have KY or compact license
FLU Vaccine or approved ARH exemption
Required for a RN specialties, except OR
Copies of exams needed, within year of start with passing score of 80% or higher
2 references required; 1 Charge or Higher; 1 must be supervisor reference
Background checks must be within 30 days prior to start.
Unit Specific Information
Unit specific to the contractor’s scope of practice within one year prior to start date; renewed annually
