Duties and Responsibilities
- Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
- Identify and re-assess patients' physical, psychosocial, and environmental needs every 15 days.
- Develop and implement individualized plans of care in collaboration with the patient, family, and attending physician.
- Document accurate and relevant clinical notes regarding patients' conditions.
