">Role Overview
">This full-time RN Case Manager position is an excellent opportunity for a skilled nurse to join our team. As an RN Case Manager, you will assume full nursing responsibility for delivering the Plan of Care for each hospice patient, continuously evaluating personal and professional performance to increase productivity and quality of care.
">Responsibilities
">- Make initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
- Evaluate patients' physical, psychosocial, and environmental needs, reassessing as needed every 15 days.
- Initiate and coordinate the plan of care, maintaining accurate and relevant clinical notes regarding patients' conditions.
- Document problems, goals, interventions, and patient/family response to hospice care.
- Collaborate with patients, families, attending physicians, and IDG members to deliver daily patient care.
Requirements
">To succeed in this role, you must possess:
">- One year of clinical experience.
- Current RN licensure in the state of practice and one year of clinical experience.
- Current CPR Certification.
- A valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
