Job Title: Vocational Case Manager
Reports To: Program Manager
Position Type: Full-Time, Non-Exempt Grant-Funded
Location: Round Mountain, CA
Schedule: Monday – Friday 8:20am to 5:00pm
Job Summary:
The Vocational Case Manager provides comprehensive vocational support services to patients with behavioral health and medical conditions, assisting them in achieving employment and educational goals. This role integrates case management, job readiness training, and collaboration with community resources to support workforce development.
The Case Manager will work directly with participants, conducting assessments, developing participant driven care plans, and coordinating training and wraparound services. This role requires collaboration with both internal program staff and external community partners to meet participant needs and ensure continued engagement throughout the training process.
Key Responsibilities:
Participant Assessment & Evaluation
- Conduct individualized assessments to evaluate participants’ needs, career interests, and needs.
- Analyze assessment data to develop individualized care plans with participant that aligned with their goals.
- Meet with participants to ensure accurate understanding of their needs and career aspirations and readiness for training.
- Determine eligibility based on assessment results and available resources.
Training & Workforce Development Support
- Refer participants to relevant training programs, workshops, and educational opportunities that match their skills and interests.
- Provide ongoing support service and or collaborate with other agencies to provide childcare, housing assistance, transportation, to provide a better outcome.
- Monitor participant progress, adjusting Care plan as necessary to accommodate changes, milestones, and needs of participant.
- Collaborate with staff to ensure seamless transition from training to employment.
Service Coordination, Referrals, Compliance & Development
- Serve as the primary liaison between participants and community service providers.
- Ensure timely and accurate documentation of participant engagement, referrals and interactions.
- Oversee participant engagement in wraparound services, coordinating resources to address personal and professional barriers to success
- Maintain thorough and accurate case notes, service logs, and documentation of participant progress.
- Adherence to reporting requirements by providing regular updates to the Program Manager.
Qualifications & Requirements:
Education & Experience
- Bachelor’s degree in social work or related field preferred. Associate’s degree or minimum of 3 years of related experience in vocational rehabilitation, social services, case management, workforce development, or a related field.
- Previous experience with EHR, assessments, care plans, capturing information and data needed to meet program criteria.
- Previous experience in an FQHC or CCBHC is highly desirable.

- Excellent communication and soft skills to build relationships with participants, service providers, and program staff.
- Strong case management and counseling skills.
- Ability to engage and support diverse populations, including individuals with mental health and substance use disorders.
- Compassionate, solution-focused approach to working with participants facing personal and professional challenges.
- Ability to work both independently and as part of a team to achieve program goals.
Additional Requirements:
- Valid California driver’s license.
- Background check and TB test clearance.
Work Environment & Conditions
- This position will require travel for participant engagement, meetings, workshops etc. Starting from the Round Mountain location, Hill Country will provide transportation services for clients, ensuring their access to essential resources.
- Hybrid work environment may be available based on program needs.
- Flexibility in schedule may be required to accommodate participants' schedules and after hour activities.
