Job Title: Full-Service Partnership (FSP) Adult Case Manager
Department: Behavioral Health
FLSA Status: Non-Exempt
Reports To: Director of Case Management
Hours: Monday through Friday 8:30am to 5:00pm
Location: Round Mountain
Position Summary:
The Adult FSP Case Manager provides intensive case management, advocacy, and support services to at-risk clients focusing on fostering resilience, stability, and mental health recovery. This role coordinates a range of services tailored to each individual’s needs, including mental health support, educational guidance, family intervention, and resource connection. The primary goal is to enhance the individual’s ability to succeed in their community, foster independence, and ensure safety and well-being.
Key Responsibilities:
- Comprehensive Assessment and Case Planning
- Conduct thorough assessments to understand each client’s needs, strengths, and challenges, considering social, emotional, and developmental factors.
- Develop individualized case management and treatment plans in collaboration with the youth, their family, and relevant care providers.
- Set realistic goals that promote personal growth, stability, and recovery for each client.
- Support Services
- Provide referrals to one-on-one counseling, support, and mentorship, helping them address behavioral, mental health, and social challenges.
- Collaborate with clients to build a supportive home environment, offering education on mental health, crisis intervention, and other resources.
- Conduct regular check-ins to monitor progress, celebrate achievements, and adjust plans as necessary to meet evolving needs.
- Service Coordination and Resource Navigation
- Connect clients with mental health services, housing resources, employment training, and other community support.
- Serve as a liaison between client and other agencies to ensure coordinated care and resource accessibility.
- Develop and maintain relationships with community partners to expand the network of services available to clients.
- Crisis Intervention and Safety Planning
- Respond to crises by providing immediate support, de-escalation, and referrals to emergency services as needed.
- Create and implement safety plans tailored to the client’s environment, considering risk factors like self-harm, homelessness, or family instability.
- Work with local crisis response teams and mobile support units to ensure rapid intervention when necessary.
- Documentation and Reporting
- Maintain a daily accurate, confidential records of client interactions, progress notes, and outcomes in compliance with Hill Country, collaborative partners and legal standards.
- Prepare reports on client outcomes, case management activities, and program effectiveness for internal and external stakeholders.
- Ensure all documentation meets regulatory, HIPAA, and funding guidelines.
Qualifications:
- Bachelor’s degree in Social Work, Psychology, Counseling, or a related field preferred or equivalent work experience.
- Minimum of 2 years of experience working with at-risk clients, case management, or social services, ideally in a Full Service Partnership or similar wraparound program.
- Strong understanding of trauma-informed care, behavioral health, and community resources.
- Excellent interpersonal, communication, and organizational skills.
- Ability to work independently, travel within the community, and adapt to varied work environments.
- Must have current and clean CA driver’s license.
Preferred Skills:
- Bilingual (Spanish/English or other languages relevant to the area).
- Familiarity with mental health diagnoses, substance use, and behavioral issues affecting clients
- Experience in crisis intervention, wraparound services, and team-based care.
SUPERVISORY RESPONSIBILITIES:
This job has no direct supervisory responsibilities, however is responsible for delegating tasks to visit coordinator and will provide feedback on support staff performance to Behavioral Health Director or other supervisors as requested.

Language Ability:
Must have excellent communication skills. Ability to speak publicly and one-on-one in an effective and persuasive manner. Ability to read, write, analyze, and interpret complex documents. Ability to respond effectively to the most sensitive inquiries or complaints.
Math Ability:
Must have intermediate math skills, sufficient enough to help clients develop budgets and manage personal finances. This includes basic abilities to add, subtract, multiply, and divide.
Reasoning Ability:
Must have very high reasoning skills. Ability to assess mental status and determine needs for emergency interventions or other forms of care. Ability to define problems, collect information, establish facts, and draw valid conclusions.
Computer Skills:
To perform this job successfully, an individual should have knowledge good working knowledge of Windows operating systems, Electronic Health Records, Microsoft Office Suite - Word, Excel, Outlook, and Internet browsers and related software.
Equipment Skills:
Proficient with standard office equipment including computers, copiers, fax machines, printers, multi-line telephone systems. Must be an experienced driver and comfortable driving with passengers.
