Job Description:
Help patients achieve their health/wellness goals and improve clinical outcomes by providing guidance, support, education, and coaching on lifestyle changes and disease state management.
Responsibilities and Duties:
- Work in an interdisciplinary team (eg, physicians, nurses, pharmacists, navigators, community health advocates) to help patients manage existing conditions, reduce utilization, and/or improve outcomes.
- Visit physician offices as needed and maintain strong relationship with physicians/practice staff to enhance care coordination and promote HE program.
- Provide patient education telephonically or in-person, in one-on-one sessions and via group classes on topics including but not limited to:
- Diabetes
- Hypertension
- Nutrition
- Weight management
- Exercise planning
- Stress management
- Proactively outreach to high-risk patients for chronic disease management.
- Develop and implement a personalized, comprehensive health improvement coaching plan based on member’s goals.
- Assist in closing quality care gaps (e.g., breast cancer screening, colorectal cancer screening).
- Maintain client privacy, safety, confidentiality, and advocacy while adhering to ethical, legal, regulatory standards.
- Demonstrate strong interpersonal communication skills, i.e.: motivational interviewing.
- Actively engage and build rapport with members and/or their support systems.
- Ability to work independently with self-initiative and discipline.
- Uses technology platforms to identify outstanding clinical care gaps (e.g., preventive screenings) and works with patient to ensure that services/labs are completed.
- Maintain accurate and timely documentation in the electronic health record.
- Minimum of a bachelor’s degree in public health, health education, or related field.
- Minimum of 3 - 5 years of experience as a health educator/health coach.
- Experience and content competence in one or more areas including: chronic conditions management, health promotion, women’s health, or counseling/behavior change.
- Experience promoting new ideas, supporting and managing change efforts.
- Strong oral and written communication skills, preferably applied towards motivating providers, practice staff and patients to adopt behavior changes.
- Strong organizational skills.
- Ability to effectively manage multiple tasks and respond to changes in organizational initiatives.
- Ability to problem solve and make decisions.
- Must be flexible and able to adapt to changing patient and organizational priorities
- Ability to travel and attend meetings that may occur outside of normal office hours.
