What are the responsibilities and job description for the Regional Case Manager position at Hill Valley Healthcare?
Hill Valley is looking for a dynamic regional case manager to oversee the Managed Care stays of residents in multiple skilled nursing facilities in the Seattle Washington area.
I want a regional team member who is a strong patient advocate. We prefer a nurse with MDS experience who understands the work flow associated with case management, but will train any licensed professional with long term care experience such as therapists, nurse practitioners or licensed social workers. I am looking for a self-started and a quick learner. I am also looking for a leader and a strong member of a regional team.
This position will work with facilities in their area to respond to insurance case managers and with concurrent reviews of managed care stays. This is a remote position and requires strong computer skills including knowledge with Net Health, Point Click Care and Microsoft Office. This is not related to hospital case management so experience in that area does not apply to this position.
This position will report to the Director of Case Management in supporting centers in need.
A current RN / LPN / PT / OT / SLP or LSW nursing license to practice in the state you reside or a compact license is required.
A working knowledge of Medicare and Managed Care regulations required. A PASSION for long term care and resident advocacy is also required and the desire to work with teams of interdisciplinary professionals.
