Hill Valley is looking for a dynamic regional case manager to oversee the Managed Care stays of residents in multiple skilled nursing facilities in Nashville, TN area.
We are in search of a dynamic regional team member who is a passionate patient advocate. This exciting position invites you to partner with facilities in your area, working closely with insurance case managers and conducting concurrent reviews of managed care stays.
Please note that this role is distinct from hospital case management, so previous experience in that area is not applicable. You will report directly to the Director of Case Management, providing invaluable support to centers in need.
Ideal Candidates Will Possess :
A current nursing license (RN / LPN / PT / OT / SLP) or a licensed social worker (LSW) to practice in your state, or a compact license, is essential for this position.
While our preference leans toward a nurse with MDS experience and an understanding of the case management workflow, we are excited to provide training for any licensed professional with long-term care experience, including therapists, nurse practitioners, or licensed social workers.
A solid understanding of Medicare and Managed Care regulations is crucial, along with a genuine passion for long-term care and resident advocacy.
An eagerness to collaborate with interdisciplinary teams, making a meaningful impact in the lives of others.
The ability to be a self-starter who is a quick learner and possess a natural leadership ability.
Strong computer skills, including proficiency in Net Health, Point Click Care, and Microsoft Office.
