What are the responsibilities and job description for the PIWC- DC - Recovery Support Specialist position at Hillcrest Children Center?
Position Summary:
The Recovery Support Specialist (RSS) in the PIWC-DC program provides direct peer support to participants, empowering them to actively engage in their care plans and achieve recovery goals. The RSS plays a key role in addressing social determinants of health and promoting participant self-advocacy through mentorship and advocacy.
Duties and Responsibilities:
Client Engagement and Support:
- Conduct regular assessments of participants’ needs, goals, and progress.
- Build trusting relationships with participants to support engagement in care and adherence to treatment plans.
- Provide mentoring on symptom management, coping strategies, and self-advocacy skills.
Treatment Adherence:
- Assist participants in attending medical, behavioral health, and social service appointments.
- Support adherence to care plans, including medication schedules and therapy participation.
- Address barriers to treatment, such as transportation or logistical challenges.
Community and Social Support:
- Help participants establish support networks through community organizations and self-help groups.
- Assist with applications for government benefits (e.g., SSI, Medicaid) and securing housing.
Monitoring and Reporting:
- Document interactions and progress within 24 hours of service provision.
- Provide regular updates to the Care Coordinator on participant progress and challenges.
- Maintain data and reports required for compliance with program guidelines.
Training and Professional Development:
- Complete required trainings on trauma-informed care, HIPAA compliance, and geriatric health.
- Participate in team meetings to provide feedback and receive support for professional growth.
Qualifications
- Bachelor’s degree in Human Services or related field required; Master’s preferred.
- Peer support certification or lived experience in recovery strongly preferred.
- Strong communication and advocacy skills.
- Knowledge of community resources and social service systems
