What are the responsibilities and job description for the Wellness Coordinator LPN/RN PRN position at Hillsboro Area Hospital Inc?
An RN/LPN Wellness Coordinator is a part of the overall commitment by the staff to provide a high-quality resident-focused plan of care, including the care for the physical, social, mental, emotional, and spiritual needs of the resident. This position is responsible for the Resident Medication Program, Resident service plans, in servicing of staff and ADL assessments. Works closely with the Executive Director and Operations Care Coordinator to maintain the health and wellness of the residents of Tremont Ridge. Scheduled workdays and times could vary and may include weekends and holidays.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Supports and promotes an environment conducive with the Mission, Vision, and Values of the hospital.
- Assures all staff members are competent to fulfill their assigned responsibilities.
- Supports and participates in the resident centered activity program.
- Performs assessments and reassessments to determine residents’ needs and establish appropriate service plans.
- Establish and maintain a medication system which adheres to state, federal and company laws, regulations, and guidelines.
- Notifies Operations Care Coordinator, Executive Director, or designer of changes in resident’s condition in a timely and appropriate manner.
- Communicate to residents, families, hospital committees, resident services staff, and Executive Director in an effective and respectful manner as needed.
- Keep written notes of specific assignments and records pertinent information about the Residents in the Special Care Cottage and/or Assisted Living
- Correspond with Physician’s offices and Pharmacies, as necessary.
- Coordinate, monitor and document all resident’s medication under the supervision of the Operations Care Coordinator and guidelines of State regulations.
- Staff education
- Acts as a contact for issues related to resident care within the community.
- Ensure adherence to company policy and State Health and Safety Codes, and federal regulations for assisted living communities.
- Maintain confidentiality of all patients, hospital, or physician related information
- Maintain orderly condition of assigned work area.
- Is knowledgeable of general hospital policies and procedures.
- Provide educational training & materials for staff every 30 days.
- Maintain resident records with accurate medication lists, service plans, and assessments.
- Reporting to IDPH when Executive Director is off.
- Companion scheduling
- Maintain accurate documentation of all variance reports and timely reporting to Executive Director and hospital as needed.
- Participate in on-call rotation schedule.
- Other duties may be assigned and are subject to change with or without prior notice.

- Weekend rotation.

Requirements:EDUCATION AND/OR EXPERIENCE
- Must be able to read, write and communicate in English.
- Graduation from high school or GED is preferred.
- Must have the ability to follow written and verbal directions.
- Graduate of accredited school of license practical nursing
CERTIFICATES, LICENSES, REGISTRATIONS
- Hold and maintain RN or LPN Illinois Licensure
- CPR certification
- In compliance with Criminal Background Check requirements
PHYSICAL DEMANDS
- Prolonged, extensive, or considerable standing and walking
- Required to climb, balance, stoop, and kneel.
- Required to use arms, hands, and fingers for repetitive grasping, pulling, and pushing and feeling.
- Frequently lift and/or move up to 25 pounds and occasionally lift and/or move up to 50 pounds, any workload exceeding 50 pounds will require assistance.
- Specific vision abilities required by this job include close vision, distance vision, color vision, and ability to adjust focus.
WORK ENVIRONMENT
- Works in well light, climate-controlled setting
- May be exposed to communicable diseases, unpleasant sights, odors, and materials.
- The noise level in the work environment is quiet to moderate.
