POSITION SUMMARY

Relocation Assistance available up to $15,000

 

Pay range: $76,731.20/annually - $111,259.20/annually

 

The Behavioral Health Care Manager (BHM) works as a key member of the collaborative care team within a Primary Care clinic. Supports and coordinates the mental and physical health care of an assigned caseload of primary care patients, collaborating with the patient’s primary care provider, psychiatric consultant, and other mental health providers when necessary. Contributes to a positive work environment that promotes HMC values. Must have current license as Licensed Clinical Social Worker (LCSW), or Licensed Psychologist (PsyD), or Licensed Professional Counselor (LPC), or Licensed Marriage & Family Therapist (LMFT), or Clinical Social Work Associate (CSWA) in the State of Oregon.

 

Patient Support and Coordination:

Working directly with patients to help manage both mental and physical health needs, supporting patients in navigating their care, ensuring follow-up on appointments, providing patient education about disorders and treatment options, and managing collaborative care registry.

 

Behavioral Health Interventions:

Screening and assessment of patients for common mental health and substance use disorders, provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate.

 

Manage treatment plans:

Systematically track treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications, facilitate treatment plan changes for patients who are not improving as expected, facilitate referrals for clinically-indicated services, develop and complete relapse prevention self-management plans with patients who have achieved treatment goals.

 

Caseload Management:

Participate in regularly scheduled caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient’s medical provider, collaborates closely with the primary care provider (PCP), psychiatric consultant, and other mental health professionals to ensure patients receive comprehensive care that integrates both mental and physical health aspects.

 

JOB SPECIFICATIONS

Education & Experience

 

REQUIRED: 

  • Graduate degree in social work or related field.
  • Three (3) years of work experience preferably in a health care setting providing social work, mental health and staff supervisory services.
  • Behavioral health counseling and brief short-term therapy strategies and case management in an ambulatory setting.
  • Experience with program development and evaluation.

PREFERRED: 

  • Master’s in social work or Ph.D./Psy.
  • Experience or education in Collaborative Care.
  • Training in Primary Care Behavioral Health (example- University of Washington AIMS Center).

Licenses & Certifications

REQUIRED: 

  • Must have current license as Licensed Clinical Social Worker (LCSW), or Licensed Psychologist (PsyD), or Licensed Professional Counselor (LPC), or Licensed Marriage & Family Therapist (LMFT), or Clinical Social Work Associate (CSWA) in the State of Oregon.

PREFERRED: 

  • Licensed Clinical Social Worker (LCSW) or Licensed Psychologist in the State of Oregon.

 

Required Knowledge, Skills, and Abilities

 

Knowledge of:

  • Principles and practices of behavioral health consultation, including assessment, diagnosis, treatment planning, cognitive-behavioral counseling strategies and their application to population-based care for preventive and chronic care issues.
  • Evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders, when appropriate (e.g., motivational interviewing, problem-solving treatment, behavioral activation).
  • Comprehensive understanding of the interrelationship between medical and psychological systems and their impact on health.

 

Skill in:

  • Effective written and verbal communication skills to interact with staff, patients, and providers.
  • Conducting group and individual psycho-educational classes and brief, short-term therapy strategies that will help strengthen the patient's overall physical and mental health.
  • Efficiently organizing time so that unscheduled, same day services are routinely available to care team members.
  • Developing effective working relationships with peers, supervisors and professionals in related disciplines.
  •  

Ability to:

  • Utilize assessment information and collateral information to conceptualize patient psychopathology.
  • Exercise sound judgment in appraising situations and making decisions regarding complex physical and behavioral health treatment issues in consultation with the primary care teams.
