Schedule: Part-Time, days, 24 hrs/wk
Pay Rate: $23.40 - $37.22 per hour
Pay Grade: 10
Position Overview:
Reports to the Director of Social Services, responsible for providing social and emotional support/services to residents and families. Completes psychological assessments and ongoing MDS assessments, care planning and progress notes. Must have experience with long term care, skilled care, knowledge of Medicare, Insurance and Medicaid regulations and procedures and discharge planning.
General Responsibilities:
Ability to work as a team and independently, have flexibility, knowledge of nursing home regulations and procedures. Ability to work effectively with nursing home staff, residents, families and community agencies. Must possess a positive disposition, be highly motivated and have an empathetic approach with social work responsibilities.
Qualifications:
Bachelor’s degree preferably in Social Work, Psychology or related field with 1-3 years’ experience in long term and skilled care experience in social service. Must be proficient in computer use, electronic medical records and Point Click Care is preferred.
