Qualifications:
- Possession of a current Texas LVN or RN license.
- Proficient in conducting clinical assessments and applying critical thinking.
- Exceptional communication skills with an emphasis on teamwork and cooperation.
- Valid Basic Life Support (BLS) certification.
- Familiarity with electronic medical record (EMR) platforms, including PointClickCare, and general computer proficiency.
Responsibilities:
- Conduct thorough evaluations of each resident’s health to create customized care plans that address their individual needs.
- Administer medications and treatments in accordance with established guidelines, ensuring the highest level of resident safety and comfort.
- Continuously assess and revise care plans to accommodate any changes in a resident’s condition, aiming for optimal health outcomes.
- Provide education to residents and their families on managing health, exploring treatment options, and implementing preventive care for informed choices.
- Collaborate with physicians, therapists, and other healthcare professionals to deliver integrated, all-encompassing care.
- Ensure precise and complete documentation of nursing actions and resident progress to meet regulatory requirements.
- Adhere to facility policies and state/federal regulations, upholding the utmost quality of care.
