What are the responsibilities and job description for the Phlebotomist/Lab Assistant position at Hillside Medical Office?
Duties & Responsibilities:
- Patient Care:
· Educates and prepares patients for laboratory test, provides emotional support and reassurance as needed.
· Positions and immobilizes patients for phlebotomy in a manner that maximizes accuracy and minimizes discomfort and injury.
· Obtains specimens from patients appropriate for tests ordered phlebotomy
· Properly labels all specimens in compliance with CLIA regulations.
· Prepares specimens for in-house testing.
· Prepares specimens and packages for shipment to reference lab(s).
· Prepares reagents and necessary equipment for the performance of tests.
· Performs in-office laboratory testing on specimens of blood, urine, sputum, feces, etc., as outlined in manufacturer's instructions and in accordance with commonly accepted lab principles and practice guidelines.
· Utilizes standing orders established by the practice for preventive and chronic care conditions in delivering patient care and education.
· Flags abnormal test results and bring to the attention of the provider.
2. Documentation:
· Verifies and/or obtains documented orders including diagnosis for all tests performed.
· Obtains patient histories appropriate for laboratory tests and documents in accordance with documentation guidelines established by the practice, meaningful use measures established by CMS, CLIA regulations and/or designated quality assurance programs.
· Maintains logs or records of all laboratory tests performed.
· Maintains patient records and test results.
3. Patient Communications:
· Notify patients of test results and provide necessary follow-up per office protocol.
· Return calls and/or electronic messages to patients within specified timeframes established by practice.
· Document clinical advice within the electronic medical record within timeframes established by practice and in accordance with documentation guidelines and medical standards of care.
· Provide patient education and self-management resources to patients and/or their care giver(s).
- Care Coordination:
- Transports patients to and from laboratory department.
· Monitors outstanding laboratory reports for past due test results and follow-up with the appropriate facility to obtain the result in accordance with timeframes established by the practice.
· Coordinates scheduling laboratory procedures performed outside the practice.
- Coordinates pick-up of specimens by reference lab.
- Distributes laboratory reports to providers.
- Quality Improvement:
- Calibrates laboratory equipment, documents and maintains quality control records in accordance with practice guidelines, CLIA regulations and/or designated quality assurance programs.
- Maintains lab supply inventory control.
· Stocks and orders laboratory supplies.
- Verifies that laboratory equipment is in good working order, performs periodic maintenance and reports equipment malfunctions as directed.
- Cleans and sterilizes lab areas, equipment and instruments.
- Maintains MSDS sheets for all chemicals in laboratory area.
- Maintains patient confidentiality at all times.
· Participates in daily huddles with assigned care team, department and/or all staff meetings.
- Attends special training sessions based on assigned job duties.
- Develops and documents protocols related to his or her job functions.
· Participates in the clinic's quality improvement activities by participating in PDSA groups, providing feedback, reporting incidents, assisting with workflow redesign, collecting and reporting assigned performance indicators, and Quality Improvement Plan (QIP) review and evaluation activities.
- Performs other appropriate duties as requested by supervisors.
Performance Requirements:
1. Knowledge of the principles and skills needed to provide patient care and treatment.
2. Knowledge of laboratory principles, quality control, CLIA guidelines, anatomy, medical terminology and methods and techniques to perform laboratory testing.
3. Knowledge of safety hazards common to a medical laboratory including OSHA regulations, universal precautions, blood-borne pathogens, and chemical safety.
4. Skilled in using various types of equipment for laboratory testing (e.g. hematology, immunology, chemistry, urinalysis, pipettors, bunsen burner, centrifuge, incubator, label maker, etc).
5. Skilled in establishing and maintaining effective working relationships with patients, clinic staff and the public.
6. Ability to perform laboratory tests relating to the analysis of blood, urine, sputum, feces, etc. and discern accurate test results from inaccurate test results.
7. Ability to apply regulatory guidelines and maintain quality control standards.
8. Ability to spell medical, surgical and pharmaceutical words correctly.
9. Ability to recognize problems and recommend solutions.
10. Ability to react calmly and effectively in emergency situations.
11. Ability to interpret, adapt and apply guidelines and procedures.
12. Ability to perform assigned duties with patient care at the center of all decision making.
13. Ability to perform duties accurately and in an efficient, organized manner.
14. Maintains appropriate certification(s).
