Job Summary

The Community Asset Navigator I is responsible for connecting individuals with resources necessary to meet their needs whenever possible. In addition, this position is responsible for developing and maintaining strong community partnerships with professional and natural supports to assist families and youth in meeting their needs and forming community connections. The ultimate focus of the Community Asset Navigator I is to improve access to resources for individuals and families with a goal of establishing self-sufficiency; in support of preventing, reducing, or eliminating involvement with the Juvenile Justice System.

Reinvesting In Youth (RIY) is a new program Hillside is excited to be starting.  The program will provide intensive, home and community based services to at-risk youth (12-18) and their families in Monroe County.  Experience in Juvenile Justice a plus.

Essential Job functions

  • Engage individual and family in the community; linking individuals with natural resources and existing programming to help ensure that individuals and their family will have strong, ongoing support.
  • Establish trusting relationships with individuals while promoting general support and encouragement.
  • Assist individuals through successful navigation to needed resources.
  • Work in partnership with interdisciplinary team to support individuals and their family with identified needs and goals.
  • Maintain complete and working knowledge of community resources available to individuals.
  • Create and maintain records of available resources and instructions for connecting community members with these resources in a resource manual. 
  • Maintain electronic medical record (EMR).
  • Participate in orientation and training of new staff.
  • Participate in team meetings and offer meaningful constructive feedback to improve teamwork and effectiveness of the program at meeting the needs of the community.
  • Establish and maintain effective interpersonal relationships with community-based organizations, clients, and co-workers.
  • Assist with community education, enrollment sessions, and other community events as requested.
  • Work with the Project Rise Steering Committee, Neighborhood Centers, co-workers, and community partners to identify opportunities to advance service of the community.
  • Attend additional community meetings as assigned.
  • While this job description covers many aspects of the role, employees may be required to perform other duties as assigned.

Education & Experience

  • High school diploma or GED required. 
  • Must be familiar with the unique culture of the neighborhood.

SPECIAL REQUIREMENTS

  • Unrestricted, valid NYS driver’s license for minimum of 1 year with a clean driving record and minimum insurance coverage that meets agency standards.

  • Knowledge of internal and external community services to build and sustain health lives.
  • Ability to advocate for new or existing services while maintaining confidentiality and professional boundaries.
  • Ability to complete documentation in electronic medical record (EMR).

Physical Demands & Work Environment

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • Must be able to work a variable scheduling including evenings and weekends
  • The following daily physical demands are required:
    • Sitting (up to 6 hours)
    • Occasional standing (up to 2 hour)
    • Occasional walking (up to 2.5 hours)
    • Driving (several times a week up to 6 hours)
    • Continuous balancing (up to 8 hours)
    • Occasional balancing, bending, stooping, climbing, kneeling, pushing, pulling, reaching forward or down, reaching overhead, running, and twisting (up to 2.5 hours)
    • Weekly lifting up to 10-15 lbs. on a daily basis
    • Manual dexterity is required, including the frequent ability to grasp in both hands and continuous use of fine manipulation skills in both hands (approximately 1-2.5 hrs.)
    • Occasional exposure to dust, fumes, gases, chemicals, or smoke is apparent  
    • Ability to change positions as needed

SPECIAL CONSIDERATIONS

While Hillside is a restraint free environment, there may be times in a “life or limb” situation, that staff may be required to physically restrain clients weighing between 50 lbs. and 300 lbs. and guide them safely to the floor. In some circumstances, restraints can last up to 15 minutes and may require repetition as necessary.  

 

$19.46 Minimum pay rate, $29.00 Maximum pay rate, based on experience.
