ACS Care Coordinator PT

Hillview Mental Health Center is currently recruiting for Care Coordinators for both day and night positions with special shifts requirements.

Adult Residential –

Day Shift (24hours/week): Sunday,Friday,Saturday – 7:00am to 3:30pm

Swing Shift: (24 hours/week): Sunday to Tuesday OR Thursday-Saturday – 3:00pm to 11:30pm

Adult Residential is a short–term 24 hour program with capacity for 15 residents.

COVID-19 considerations: All employees must be fully vaccinated. All employees and clients are required to wear masks within the premise and social distance when possible.

Required Skills:

• 1-2 years experience/knowledge related to working in the mental health field.
• Proficient with written and oral communication skills.
• Must have basic computer skills, specifically Microsoft Word and Excel.
• Must have basic typing skills
• Comfort working with a diverse clinical population, including clients with severe and persistent mental illnesses as well as clients referred from the criminal justice system.
• Comfort working independently and with a multidisciplinary team in a fast-paced environment.
• Ability to maintain productivity and documentation standards.
• Compassion for clients.
• Ability to manage time efficiently, prioritize tasks, demonstrate sound judgment, give attention to detail, be flexible, self-driven and motivated.

Preferred Skills:

• Previous case management experience.

Qualifications:

• Associate's Degree in mental health or Bachelor's Degree
• Must show valid California driver's license and current automobile insurance prior to start date, and thereafter, on an annual basis.
• Must maintain a good driving record.
• Ability to pass thorough background check.
