Hinds Hospice, one of the most well-known and well-respected nonprofit agencies in California’s Central Valley and a pioneer in the practice of hospice and palliative care, is seeking a Full Time RN Case Manager to join our team.
The RN Case Manager is an experienced Registered Nurse who provides skilled palliative and end-of-life care in the community setting. As a member of the Interdisciplinary Team, the RN Case Manager facilitates the development of a collaborative interdisciplinary plan and ensures the overall coordination, collaboration and medical management of hospice services for patients and their significant others.
The RN Case Manager is also responsible for promoting the philosophy of hospice, which redefines hope by focusing on quality of life and connection with loved ones during a patient’s final days, rather than on curing a disease.

Qualifications
- Compensation $55 - $57 an hour

Qualifications for this position include but are not limited to:
- Completed a professional nursing program accredited by the National League for Nursing (prefer completion of a baccalaureate program in nursing).
- One year experience in acute care facility or community health agency, (hospice experience a plus).
- Current RN Licensure and CPR Certification.
- Valid driver’s license, auto insurance, reliable transportation and a willingness to use personal vehicle in the course of employment.
- Ability to meet the physical demands of the job (i.e., Push, Pull, Lift/Carry 35 lbs.)
- Availability to rotate weekends.
