Job Summary:
We are seeking a skilled Registered Nurse to join our team as a Chronic Condition Case Manager RN. In this role, you will be responsible for providing individualized coaching to patients with chronic medical conditions, promoting self-management and improving health outcomes.
About the Role:
- You will assess patient needs and develop a plan to meet their healthcare requirements, working closely with physicians, providers, and payers.
- Utilize your clinical expertise to identify problems and determine economically responsible options for patient care.
- Collaborate with healthcare teams to facilitate quality, timely, and efficient medical care across the continuum.
- Implement disease management plans, coordinating resources and evaluating their effectiveness.
- Promote patient self-management through education and motivational interviewing principles.
- Demonstrate excellent communication skills, negotiating effectively with stakeholders and maintaining confidentiality.
- Provide written documentation of patient information, treatment plans, and disease management interventions.
Requirements:
- Current unrestricted license to practice as a Registered Nurse in the state or reciprocating state.
- Successful completion of disease management orientation program.
- Minimum 2 years full-time equivalent direct clinical care experience.
- Excellent communication and typing skills (35 WPM minimum).
- Previous disease management or case management experience is helpful but not required.
