- Evaluation and Outcomes Measurement
- Evaluate the effectiveness of disease management plans and make modifications as necessary.
- Monitor and measure patient outcomes to ensure the highest level of care.
- Patient Education and Support

Requirements
- Current unrestricted license to practice as a Registered Nurse in the state where Hines & Associates Inc operates.
- Minimum of 2 years of direct clinical experience in caring for patients with chronic conditions.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Basic computer skills and proficiency in typing, with a minimum speed of 35 words per minute.
